Selecting a Medical Care Provider for your Child
Town & Country Pediatrics is a group practice. Choosing us for
your child's medical care enables you to have many options. You
can choose to see a board-certified pediatrician or nurse practitioner
with doctorates. Some of our providers stay at one location site
and others have regular schedules at two of the three offices. Our
Provider CV's and photographs are available
in the All About Us section. If you still
have questions about choosing a provider, or want more information

administrator at 773-929-2260. She will be able to help match you
up with a provider who can meet the needs of you and your child.
Town & Country Pediatrics does not give pre-natal interviews.
New Patient: Newborns
If you are ready to deliver a baby and want to choose Town &
Country Pediatrics for your child's medical care, you do not have
to pick a specific provider at this time. Let your obstetrician
and the admitting office at one of our delivery hospitals (Prentice
or Evanston) know that you have chosen Town & Country Pediatrics.
The hospital will notify us within the hour of your child's birth.
A Town & Country physician will visit you and your child the
morning following your child's birth. Whichever physician is on
call that day will examine your child and stop in to see you. A
Town & Country physician will examine your child each day he/she
is in the hospital. These charges will be separate from your hospital
bill. It is your responsibility to call the Insurance Office at
773-929-2260 as soon as your child has been added to your insurance.
Until this information is received, you will be responsible for
your bill. Our physician will talk to you each day you are in the
hospital. You will receive a blue booklet filled with helpful medical
advice for you and your newborn. You will also receive a pink booklet
with information about the practice. Our physician will tell you
when you need to bring the baby in for his/her next appointment.
We strongly advise you to call immediately for your appointment
and once you start coming in for your child's healthy appointment
(see Healthy Child Appointment Schedule), make your future appointments
when you check out. Our receptionists will be happy to work with
you to find the provider and the location you choose.
New Patient: Transfer from Another Physician
New patients are very important to us, so we have a few guidelines.
We set aside 30 minutes for each new patient, with a maximum of
two new patients per family, per day. If you have three or more
children, we will have to schedule them for another day. New patients
need to be here twenty minutes early for their appointment. If you
are ten minutes late, we will have to reschedule your appointment,
as the providers feel they need a full 30 minutes to examine your
child and get a complete medical history. If you cannot make your
first appointment, it is very important that you call the office.
New patients who do not show up or call to cancel their appointment
will not be allowed to reschedule. 24 hours notice is needed to
reschedule appointments.
The medical records from your past physician need to be delivered
to our office by your appointment time. You will need to sign a
release for your past physician to have your child's medical record
copied. Authorization to release medical information must be in
written form. You can do this by writing a letter requesting the
record be copied and sent to us, or you can fill out one of our
forms by clicking here. The safest way to
assure that the medical record arrives on time is to hand deliver
it at your first appointment.
All new patients must have a healthy exam before they can be seen
for a sick appointment. We advise you to make your child's healthy
exam appointment as soon as you decide to choose Town & Country
Pediatrics as your health care provider. All new patients, regardless
of their age, must be accompanied by their parent or guardian on
their first visit.
You will need to fill out a new patient information sheet for each
child, as well as a new patient record. We also need you to fill
out the Family History Questionnaire, but we only need one per family.
All of these forms can be filled out ahead of time by going to Forms
and printing out the sheets. Be sure to bring them with you on your
first visit.
We will need to make a copy of your child's health insurance card.

contracts under All About Us), as well as the guarantor's social
security number and date of birth. Once a year we will ask you to
update your patient information sheet and your insurance card. If
your insurance changes before that, please let the receptionist
know. Co-pays are collected at the time of service. We accept cash,
check, Mastercard or Visa.
