Medical Records

It is your responsibility to have previous medical records sent to Town and Country Pediatrics by your child's first visit. Hand-carrying them to your first visit may be the safest way to assure the desired results. If you need a form to fill out requesting the transfer of your medical records to Town and Country Pediatrics, please call or stop by our office or click here. We would be happy to provide you with it.

If at any time, you need your child's medical record copied for a specialist visit, we need a 7 day time frame for completion. You will need to sign a release of information sheet before we will copy the chart. Click Here for a form. We will always send copies of your child's medical records to other physicians. The original chart will remain on file at our office.

With respect to our patient's medical record confidentiality, we will not FAX any medical information other than your child's immunization record. All copies must be picked up by you or mailed.

Fill out the patient information sheet before your appointment and bring it with you. These information sheets become a permanent part of your child's medical record. Be sure to complete the entire form.
