Selecting a Medical Care Provider for your Child
Town &
Country Pediatrics is a group practice.
Choosing us for your child's medical care allows you several
options. Our offices offer extended
hours, including weekends, and multiple locations. Our

New
Patient: Newborns
If you choose Town & County Pediatrics for your child’s medical
care, inform the admitting office at one of our delivery hospitals
(Prentice or Evanston) that you have chosen Town & Country
Pediatrics. If you are delivering at another hospital, call our
office for further instructions. Check with your insurance provider
to make sure that Town & Country Pediatrics is in network under your
plan. The hospital will notify us within the hour of your child's
birth. A Town & Country physician will visit you and your child the
morning following your child's birth. A Town & Country physician
will examine your child each day he/she is in the newborn nursery.
These charges will be separate from your hospital bill. You will
receive a handout with helpful medical advice for you and your
newborn. You will also receive a green booklet with information
about the practice. Our physician will tell you when you need to
bring the baby in for his/her next appointment; usually when your
newborn is 3-6 days old. Please call the office before you leave the
hospital. You will need to schedule the newborn visit, 2 week, 1
month and 2 month visits. You may
download and complete the New Patient Registration Form and Family
History Questionnaire found on the link below. Our
receptionists will be happy to work with you to find days, times and
locations that work with your schedule. Please remember, it is your
responsibility to notify your insurance company of your child’s
birth and to add your newborn to your policy.
New
Patient: Transfer from Another Physician
New patients are very important to us, so we have a few guidelines.
All new patients, regardless of their age, must be accompanied by
their parent or legal guardian on their first visit. We set aside 30
minutes for each new patient, with a maximum of two new patients per
family, per day. If you have three or more children, we will have to
schedule them for another day. New patients need to be here twenty
minutes early for their appointment. If you are ten minutes late, we
will have to reschedule your appointment, as the providers feel they
need a full 30 minutes to examine your child and get a complete
medical history. If you cannot make your first appointment, it is
very important that you call the office. New patients who do not
show up or call to cancel their appointment will not be allowed to
reschedule. 24 hours notice is needed to reschedule appointments.
The
medical records from your past physician need to be delivered to our
office by your appointment time. You will need to sign a release for
your past physician to have your child's medical record copied.
Authorization to release medical information must be in written
form. You can do this by writing a letter requesting the record be
copied and sent to us, or you can fill out one of our forms by
clicking here.
The safest way to assure that the medical record arrives on time is
to hand deliver it at your first appointment.
All
new patients must have a healthy exam before they can be seen for a
sick appointment. Unfortunately, we are unable to give medical
advice until your child has been seen. We advise you to make your
child's healthy exam appointment as soon as you decide to choose
Town & Country Pediatrics as your health care provider.
You
will need to fill out a new patient information sheet for each
child, as well as a new patient record. We also need you to fill out
the Family History Questionnaire, but we only need one per family.
All of these forms can be filled out ahead of time by going to
Forms
and printing out the sheets. Be sure to bring them with you on your
first visit.
We
will need to make a copy of your child's health insurance card. (See
a current list of our
insurance contracts
under All About Us), as well as the guarantor's social security
number and date of birth. Once a year we will ask you to update your
patient information sheet and your insurance card. If your insurance
changes before that, please let the receptionist know. Co-pays are
collected at the time of service. We accept cash, check, Mastercard
or Visa.
