Answers to your Medical Record Questions

I am transferring to Town & Country Pediatrics from another doctor. What should I do about the medical records?
It is your responsibility to have previous medical records sent to Town & Country Pediatrics before your child's first visit. Hand-carrying them to your first visit may be the safest way to assure the desired results. If you need a form to fill out requesting the transfer, go to the Forms Section to print one out.

What do I do if my child's specialist needs a copy of your records?
Please sign a release (go to the Forms Section) and FAX, mail or drop it off at our office. Records are copied within a 7 day time frame. We will always send copies of your child's chart. We always keep the original chart in our office files. With respect to your child's confidentiality, we will not FAX copies of the medical record, other than the vaccine record. All copies must be picked up by you or mailed.
