Selecting a Medical Care Provider for your Child
Town & Country Pediatrics is a group practice. Choosing us for your child's medical care allows you several options. Our offices offer extended hours, including weekends, and multiple locations. Our Provider CV's and photographs are available in the All About Us section.
New Patient: Newborns
Our physicians are on the medical staff of Northwestern Memorial's Prentice Women's Hospital. If you choose Town & Country Pediatrics for your child's medical care, please identify one of our pediatricians on your admitting/ registration forms at your delivery hospital to ensure that our office will be notified of your child's birth. A Town & Country physician will examine your newborn each day he/she is in the newborn nursery, until the mother is discharged from the hospital. Your newborn's first office visit will typically be within 2 days following hospital discharge. Our physicians will instruct you when to make this appointment.
If you are delivering your baby at another hospital, please call our office after he/ she is born. We would like to see your newborn within a few days of hospital discharge. If possible, please request the hospital physician for all medical records pertinent to your baby's delivery and newborn care and bring a copy with you to the first office appointment.

New Patient: Transfer from Another Physician
We welcome new patients transferring into our practice and want to help make this transition as seamless as possible. All new patients, regardless of their age, must be accompanied by their parent or legal guardian for the initial visit. If possible, please bring a copy of your child's previous medical records for our providers to review. Each NEW PATIENT appointment is scheduled for 30 minutes for our providers to adequately review your child's medical history and complete a physical exam. Please plan to arrive 20 minutes prior to your appointment. We encourage you to schedule your NEW PATIENT appointment as soon as possible in order to be considered an established patient. Once established as a Town & Country Pediatrics patient, we can offer your child sick appointments. However, for your child's safety, we cannot offer medical advice over the phone until your child has been seen in the office.
Please remember to bring a copy of your child's health insurance card. We also appreciate having the Family History Questionnaire form completed prior to this appointment.
